JOB DESCRIPTION

 

TITLE:  priority populations program coordinator

 

POSITION OVERVIEW:  The Priority Populations Program Coordinator will be responsible for the comprehensive oversight and effective implementation of all components relative to the Department of Early Education and Care’s Priority Populations contract.  In addition, the Coordinator will supervise Priority Populations Social Workers to ensure individual child and family needs are addressed and further supported.

 

QUALIFICATIONS:

 

1.      Masters Degree in Social Work, Human Services, Early Education and Care or related field of study and 5 years experience working with families of Priority Populations (homeless, supportive and teen parents).

2.      Effectual supervision history for a minimum of 3 years.

3.      Implementation experience and demonstrated understanding of applicable standards, policies, procedures and regulations relative to funding sources such as:  Department of Early Education and Care, New England Farm Workers Council, United Way, etc.

4.      Outstanding oral and written communications skills.

5.      Computer proficiency skills in MS Office Suite, including:  Excel, Access, Word and Outlook.

6.      Flexibility, self-direction, organization and attention to detail in prioritizing and completing reports and tasks.

7.      Ability to work cooperatively and as a member of a team with families, colleagues, early education and care providers and the community-at-large.  

8.      Bilingual skills are preferred.  

9.      Flexible schedule to meet organizational needs.

 

DESCRIPTION OF DUTIES

 

1.      Serve as a liaison to coordinate and disseminate referrals from the Department of Housing and Community Development (DHCD), Department of Children and Families (DCF) and agencies serving teen parents.

2.      Establish prompt responsiveness to inquiries and timely enrollment for all Priority Population children.

3.      Ensure that all Department of Early Education and Care (EEC) Priority Populations contract requirements are effectively met.

4.      Develop, maintain and oversee tracking system regarding placement, slot utilization and appropriation of funding sources.

5.      Develop and maintain data system for creating required reports including, but not limited to the following program aspects:  attendance, placement, slot utilization, referred and accessed community resources, child assessment.

6.      Provide required documents in response to Square One and funder requests, in a timely manner.

7.      Conduct ongoing analysis of reported data to determine trends, challenges and new opportunities for participants in programs.

8.      Acquire current research on all aspects of priority populations to promote consistent program development.

9.      Develop and uphold positive partnerships and collaborations to effectively work with community partners, dedicated to providing comprehensive services and supports to priority population to ensure that family needs are recognized and addressed.

10.  Actively participate in and contribute to weekly Team Meetings with Center/Program Directors and Family Service Coordinators.

11.  Maintain positive, effective working relationships with all Square One staff.

12.  Coordinate comprehensive referrals and services with Square One Transportation and Family Services Department to ensure individual child and family needs are addressed.

13.  Provide leadership, supervision and support to Priority Population Social Workers.

14.  Assist Priority Populations Social Workers in initial child assessment screening, when required.

15.  Conduct weekly Priority Populations Team meetings to include case presentations for continual assessment of children’s Individual Service Plans and those activities as identified in the Individual Family Service Plans.

16.  Provide weekly program updates to the Executive Vice President of Programs

17.  Attend conferences, workshops, seminars and local/regional Priority Populations meetings as a Square One Representative advocating on behalf of Priority Population families.

18.  Other duties as assigned.

 

Physical Demand 

 

1.      Ability to stand or sit for long periods of time during working hours and on special occasions.  

2.      Ability to effectively utilize computer software programs with proven ability to have manual dexterity to maintain specific databases and reports. 

3.      Excellent auditory, verbal and sensory skills and abilities.   

 

Mental/ Visual Demand 

 

  1. High attention to detail involving normal mental and visual attention.
