JOB DESCRIPTION
Title: FAMILY SUPPORT SPECIALIST
Qualifications:
1. High School Diploma or GED is required; CADAC/Addiction Counselor Certification preferred
2. FSS will be a mother in recovery for at least two years, familiarity with 12-step group process
3. Familiar with and able to work from a strengths-based, family-centered approach.
4. Strong organizational and communication skills, including the ability to work well with a wide range of families and service providers.
5. Ability to travel within the designated areas of Hampden County as needed.
6. Basic computer proficiency skills are required to record and track activities, contacts and referrals.
7. Valid Driver’s license and use of a vehicle is required.
8. Bi-lingual/Bi-cultural strongly preferred.
9.
Knowledge and familiarity with DCF is preferred.
DESCRIPTION OF DUTIES:
1. Work cooperatively with project staff to encourage initial and ongoing participation of mothers and families in needed services.
2. Provide mentoring, advocacy, and support for pregnant and parenting mothers and fathers with substance use disorders or living with HIV/AIDS and their families through home visiting, building connections to community based services, and/or support groups..
3. Assist families with understanding and navigating the public health, medical, mental health and social service agencies that offer needed services.
4. Provide culturally appropriate support and guidance, including health related information.
5. Participate in Family Case Conferences (at DCF) and regularly scheduled supervision and project team meetings.
6. Maintain contacts with service providers and agencies, community-based resources, the family’s social network, and members of the extended family.
7. Record and track all activities, contacts, and referrals.
8. Other duties as assigned
