JOB DESCRIPTION
Title: FAMILY SUPPORT SPECIALIST
Qualifications:
- High School Diploma or GED is
required; CADAC/Addiction Counselor Certification preferred
- FSS will be a mother in
recovery for at least two years, familiarity with 12-step group process
- Familiar with and able to work
from a strengths-based, family-centered approach.
- Strong organizational and communication
skills, including the ability to work well with a wide range of families
and service providers.
- Ability to travel within the
designated areas of Hampden County as needed.
- Basic computer proficiency
skills are required to record and track activities, contacts and
referrals.
- Valid Driver’s license and use
of a vehicle is required.
- Bi-lingual/Bi-cultural strongly
preferred.
- Knowledge and familiarity with
DCF is preferred.
DESCRIPTION OF DUTIES:
- Work cooperatively with project
staff to encourage initial and ongoing participation of mothers and
families in needed services.
- Provide mentoring, advocacy,
and support for pregnant and parenting mothers and fathers with substance
use disorders or living with HIV/AIDS and their families through home
visiting, building connections to community based services, and/or support
groups..
- Assist families with
understanding and navigating the public health, medical, mental health and
social service agencies that offer needed services.
- Provide culturally appropriate
support and guidance, including health related information.
- Participate in Family Case
Conferences (at DCF) and regularly scheduled supervision and project team
meetings.
- Maintain contacts with service providers
and agencies, community-based resources, the family’s social network, and
members of the extended family.
- Record and track all
activities, contacts, and referrals.
- Other duties as assigned
